Italy introduced a ban on smoking in all enclosed public spaces (eg bars and restaurants) in 2005 and most visitors report that the ban has been successfully enforced. As in the UK, diners and drinkers have become used to going outdoors for a cigarette.
However, if the tables and chairs are outdoors then smoking is usually permitted although, as you say, it would be a good idea to check with the group before doing so, out of courtesy.
Italian hotels are allowed to set aside bedrooms for smokers, so you may be able to get one of these, where possible. If you mention it to our holiday advisors, they can make a note on your booking. If you have a room with a balcony, you should be fine smoking out there.
